  • Health screening requirements and guidelines (Updated for 2018-19)

    Effective 7/2018 - NYS is requiring all students entering K, 1st, 3rd, 5th, 7th, 9th and 11th grade as well as new entrants to have a physical completed by a NYS licensed physician. Dental certificates are also recommended to be submitted on physical required years. Please have your child's provider complete the form below and return via fax to the elementary health office at 243-8087.

    Hearing, near vision and distance acuity screenings are required of all students entering K, 1st, 3rd, 5th, 7th, 9th and 11th grade. New entrants will need color perception screening in addition. 5th and 7th grade girls are required to have a scoliosis screening. If any of these screenings are not completed by your child's physician, they will be done at the health office.

    Please note that both the grade levels and physical form that are required have changed. Due to HIPPA regulations - providers are requiring and enforcing written consent for the nurse to obtain school physicals. The best way to make sure I receive a copy is to mail one to me or drop it off at the office!

  • What to do when your child is sick

    Please notify the nurse in the case of your child's absence or tardiness by calling 585-243-3451 or email elementaryattendance@geneseocsd.org

    When bringing your child in late or picking them up early- please sign in/out in the elementary office with Mrs. Button.

  • Important Medication Information 

    Any medication that is needed to be administered in school must have a doctor's order and signed parental permission.  This includes over the counter medication such as tylenol, advil, cough drops, hydrocortisone cream and neosporin.  Below are links to medication forms for your convenience

    OTC medications such as pain relief will not be stocked by health office.  If you would like your child to have an option to take pain relief medication, please drop off an unopened properly labeled medication bottle accompanied by a physicians order.  You will have to sign paperwork upon drop off.

    Daily medications will have to be in a properly labeled bottle accompanied by a physicians order and dropped off by a parent/guardian.  Please do NOT send in medications with your child or your child's older sibling as this is an unsafe practice and not allowed.  Be prepared to sign paperwork and complete a pill count.  

    Please note that medication orders will have to be updated yearly,  This includes Epi pens and Inhalers.  ALL medication will need to be picked up at the end of the school year or it will be disposed of due to NYS law.  A letter or email will be sent as a reminder toward the end of the school year.

  • When shouldn't I send my child to school?

     If you suspect your child may be ill, please do not send them to school.  Here at Geneseo, we are committed to reducing the spread of infectious conditions and are not equipped to manage a sick child at school for any extended period of time.



    Reasons to keep your child home

    • Fever of 100 degrees or greater.  Temperature should be normal without the aid of fever reducing medication for 24 hours before they return to school
    • Vomiting and/or diarrhea in the past 24 hours
    • An unidentified rash
    • Persistent uncontrollable cough and/or chest congestion
    • Lice and/or nits (if treated properly, may return after)
    • Pink eye 
    • Painful sore throat that is persistent 
    • Significant amount of nasal drainage 
    • Severe ear pain
    • Severe headache accompanied by a fever
    • Fatigue from illness

    If your child has any illness that requires antibiotics- he/she must be on for at least 24 hours before returning to school

    Certain types of illness will require a physicians note to clear student before returning 

    Your child's physician is the best source of information and advice in regard to their health or illness
